We provide high quality, safe, efficient, and personalised support and care for people assessed by the Local Authority or CCG as being eligible for health and social care for people aged over 16

Individually tailored support
Everything we do at Rapid Improvement Supported Living Services is guided by the people who use our services, so the support we offer through ‘Supporting You’ is completely dependent on the needs of each individual.
Our enthusiastic and highly trained staff team will ensure right from the beginning that you will know what we will do to support the person, which can include helping with setting up a home and gaining a tenancy. In order to achieve the best possible outcomes, we also work in close partnership with health and social care professionals, housing, education and vocational training providers and employers

Care pathways

Our Supported Living Services works alongside education, healthcare and residential services to ensure that the support is just right for the individual.
What is Supported Living? Supported living helps you live in your own home. You get the help you need to be independent. You make choices about your life.
- We meet you (and anyone you want to bring).
- We ask: What do you want to do? What help do you need?
- Example: “I want to cook my meals” or “I need help with bills.
- Together, we write a plan just for you.
- The plan says:
- What help do you want (e.g., shopping, personal care)?
- What skills do you want to learn (e.g., using public transport)?
- You can change the plan anytime.
- You pick where you want to live:
- Stay in your current home.
- Find a new home (we help with this).
- We check the home to ensure it is safe and works for you.
- We match you with friendly staff who:
- Listen to you.
- Help you learn, not do everything for you.
- You can meet staff first and say yes or no.
- Staff visit to help with things like:
- Cooking, cleaning, or going out.
- Learning new skills.
- Joining clubs, work, or hobbies.
- Staff are there when you need them.
